Nalbuphine
Narcotic Analgesic · Opioid analgesic
Indications
Moderate to severe pain. Pre-and post-operative analgesia. Pain in suspected myocardial infarction.
Dosage
Pain: Adults: 10-20 mg by IV, IM or SC inj. Myocardial infarction: Adults: 10-30 mg by slow IV inj. A repeat dose of 20 mg may be given within 30 min, if necessary. Children: Not recommended.
Administration
Administered by SC, IM, or IV injection. For direct IV injection through an IV line with a compatible infusing solution, give drug slowly not more than 10 mg over 3 to 5 mins. Compatibility: Dextrose 5% in NaCl 0.9%, Dextrose 5%, 0.9% NaCl, LR. Incompatible with diazepam or pentobarbital. Use separate IV line or flush line well before and after administration. Stability: Stable for 48 hours when diluted in 1:1 and 1:2 ratios with diluents.
Adverse Effects
Sedation, sweating, nausea, dizziness, dry mouth.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Precautions: Respiratory depression, obstructive airways disease. Renal or hepatic impairment. Head injury. History of opioid abuse.
Interactions
CNS depressants.
Advice to Patient
If you are self-administering this medication, follow directions for administration and disposal of needles as prescribed. If constipation is unrelieved in few days, consult prescriber for suitable laxative or stool-softener. Contact prescriber if there is constant headache, acute dizziness, light-headedness, slow or fast heartbeat, chest pain, unusual weight gain, swelling of extremities, changes in mental status, changes in urinary pattern, pain on urination, back pain, muscle spasm, difficulty in breathing, skin rash, blurred vision, decrease in appetite. Avoid alcohol during therapy. Do not take OTC medications especially tranquilizers, sedatives, antihistamines, pain medications without consulting prescriber. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Change position slowly when rising from sitting or lying…

Onset of action: Peak effect: SC, IM: <15 mins; IV: 2-3 mins. Duration of action: 3-6 hrs. Metabolism: Hepatic; extensive first-pass metabolism. Half-life elimination: Children: 0.9-3.5 hrs; however, overall trend observed is longer half-life as age increases. Adults: 5 hrs. Excretion: Faeces; urine (approx. 7%).
