Indications
Urinary frequency, urgency and urge incontinence.
Dosage
ORAL: Adults: 50 mg once a day. Children: Under 18 years not recommended.
Adverse Effects
Tachycardia, UTI. Uncommon: Atrial fibrillation, dyspepsia, palpitation, pruritus, rash, vulvovaginal infection, vulvovaginal pruritus, gastritis, hypertension, joint swelling, GI upset, headache, dizziness.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Severe uncontrolled hypertension (systolic blood pressure =180 mmHg or diastolic blood pressure =110 mmHg), hepatic impairment, renal impairment (eGFR < 15 mL/min/1.73m2), haemodialysis. Precautions: History of QT-interval prolongation, stage 2 hypertension. Bladder outflow obstruction. Severe renal or moderate hepatic impairment.
Interactions
Thioridazine, anticholinergic agents, ketoconazole (systemic), mifepristone, rifampin, drugs that prolong QT interval, flecainide, propafenone, TCADs, other P-gp substrates, digoxin, dabigatran, other CYP2D6 substrates, strong CYP3A inhibitors.
Advice to Patient
Take with or without meals. Swallow whole tab with water. Do not chew, crush or break tab.
Pharmacokinetics
Onset of action: Efficacy within 8 wks, steady state achieved within 7 days. Metabolism: Hepatic. Bioavailability: Dose-dependant. 29%-35% (following 25 mg and 50 mg oral dosing, respectively). Half-life elimination: Approx. 50 hrs. Time to peak: Approx. 3.5 hrs. Excretion: Urine, faeces.
