Antidepressant · Tricyclic antidepressant (TCAD)

Indications

For relief of symptoms of depression in those cases of depressive illness where drug therapy is indicative.

Dosage

Adults: Treatment should begin with 30 mg daily and dosage should be adjusted according to the clinical response. The effective dose usually lies between 30-90 mg (mostly 60 mg) daily. Children: Not recommended.

Adverse Effects

Blood dyscrasias, convulsions, hypomania, hypotension, disturbance of liver function, arthralgia, oedema and gynaecomastia have occasionally been reported, anticholinergic effects (e.g., dry mouth, constipation, urinary retention, blurred vision). Tinnitus, confusion, drowsiness, headache, tremor, ataxia, seizures, movement disorders, dyskinesias. Cardiovascular effects (e.g., tachycardia, syncope, conduction disturbances, arrhythmias). GI upset, dysgeusia, sweating, allergic skin reactions, wt changes, libido changes, impotence, galactorrhoea, changes in blood sugar concentrations, hyponatraemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Mania, recent MI, arrhythmias, severe liver disease. Precautions: Treatment should be discontinued if jaundice or convulsions occur. If a patient shows fever, sore throat, stomatitis or other signs of infections, a full blood count should be obtained as bone marrow depression has been reported during treatment. History of epilepsy, urinary retention, prostatic hypertrophy, narrow angle glaucoma or increased IOP. Cardiovascular disorders, hyperthyroidism; monitor carefully. Elderly. Phaeochromocytoma. History of mania or psychoses. Monitor for suicidal ideation. Concurrent ECT. Porphyria. Withdraw gradually.

Interactions

Carbamazepine, phenytoin, phenobarbitone, primidone, pimozide, artemether, lumefantrine, with or within 14 days of MAOIs, alcohol.

Advice to Patient

Take with or without meals. Do not chew or crush or break tablets. Swallow whole. If you begin to feel faint, sit down and wait until the feeling passes. Do not stop taking medication abruptly, gradually lower the dose and then stop. Contact prescriber if there is anxiousness, confusion, sleeping problems, difficulty in urination, tingling, breast tenderness, altered sexual ability, changes in appetite and weight, change in taste, ringing in ears, skin rash, itching, hair loss, increased sweating, changes in heart rate or rhythm. Avoid alcohol during therapy. Change position slowly from sitting or lying to standing.

Absorption: Readily absorbed from the GI tract. Metabolism: Hepatic (first-pass; extensive). Half-life elimination: Biphasic. Terminal: 6-40 hrs. Excretion: Urine (mainly). Faeces (small amounts).
