Methylergometrine Maleate
Labour Inducer/Inhibitor · Uterotonic: Ergot alkaloid
Indications
Management of puerperium, uterine haemorrhage, uterine subinvolution.
Dosage
Inj: Adults: 0.5 mL IV or 1 mL IM after delivery of placenta. Tab: Adults: 1 tab 3 times daily for three to four days in puerperium. Children: Not applicable.
Administration
IV: Ampoules containing discoloured solution should not be used. Administer over no less than 60 seconds.
Adverse Effects
Hypertension, temporary chest pain, palpitation, dizziness, hallucinations, seizures, headache, nausea, vomiting, diarrhoea, leg cramps, hematuria, dyspnoea, nasal congestion, tinnitus.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Induction of labour, threatened spontaneous abortion, toxaemia. Precautions: Caution in patients with sepsis, obliterative vascular disease.
Interactions
Antifungals, CYP3A4 inhibitors, sumatriptan, sibutramine and other serotonin agonists.
Advice to Patient
Take with or without meals. This medication is to be used only after delivery of baby. Do not breast-feed within 12 hours of taking this medication. Contact prescriber if there is fast/slow heartbeat, difficulty in breathing, cold hands/feet, changes in vision, chest pain, seizures, confusion, redness/pain/swelling of arms or legs. Reduce or avoid alcohol intake. Avoid or reduce grapefruit product intake during therapy.

Bioavailability: Oral: 60%. IM: 78%. Half-life elimination: Approx. 3 hrs. Time to peak, serum: Oral: 0.3-2 hrs. IM: 0.2-0.6 hrs. Excretion: Urine and faeces.
