Methenamine Hippurate

Indications

Prophylactic or suppressive treatment of frequently recurring urinary tract infections when long-term therapy is considered necessary.

Dosage

Adults: 1 gm twice daily. Children =12 yrs: Same as adults. 6-12 yrs: 0.5-1 gm twice daily.

Adverse Effects

Nausea, stomach upset, dysuria and rash.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Renal impairment. Severe hepatic impairment. Severe dehydration. Precautions: Elderly.

Interactions

Sulfonamides (avoid).

Advice to Patient

Swallow whole tab with a glass of water. Do not crush, break or chew the tab. Drink plenty of water to help prevent the formation of crystals in the urine. If the medication causes an upset stomach, it may be taken with food. Consuming foods and drinks like cranberry juice, plums, prunes, and protein can help increase urine acidity. Limit the intake of milk, dairy products, citrus fruits, and most other fruits, as they can make urine less acidic and potentially reduce the drug's effectiveness. Report immediately if you experience painful/difficult urination, blood in the urine (pink/red colour), severe skin rash, hives, itching.

Onset of action: 30 mins. Absorption: Readily absorbed from the GI tract. Metabolism: Hydrolyzed to ammonia and formaldehyde in acidic urine. Half-life elimination: Approx. 4 hrs. Time to peak, plasma: 1-2 hrs. Excretion: Urine (70-90%).
