Indications
Female: Anovulatory infertility. Super ovulation for IVF. Male: Used with hCG in conjunction to stimulate spermatogenesis in those with primary hypogonadotropic hypogonadism.
Dosage
Adults: Female for induction of ovulation: 1 amp daily for 9-12 days followed by 10,000 units hCG one day after the last dose; repeated at least twice at same level before increasing dosage to 2 ampoules. Male for spermatogenesis: Following pretreatment with hCG, 1 amp 3 times/wk and hCG 2000 units twice wkly until sperm is detected in the ejaculate (4-6 mth) then may be increased to 2 amp 3 times/wk. See literature. Children: Not applicable.
Administration
IM or SC (ONLY as per manufacturers instruction) administration. The lower abdomen (alternating sides) should be used for subcutaneous administration. Stability: Lyophilized powder may be stored refrigerated or at room temperature 3°C-25°C (37°F-77°F). Protect from light.
Adverse Effects
Ovarian hyperstimulation, enlargement and rupture. Multiple pregnancy. Allergic reactions. GI upset, acne, gynaecomastia, wt gain, headache, rarely fever, joint pain.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Female: Undiagnosed vaginal bleeding, tumours of the uterus, ovaries, breasts. Male: Tumours of testes or prostate. Precautions: Treat endocrine disorders or intracranial lesions before therapy. Monitor oestrogen levels and follicular development regularly. In men exclude other causes of infertility prior to treatment.
Interactions
Clomiphene, other drugs that stimulate ovulation.
Advice to Patient
If you are self-administering this medication, follow directions for administration and needle disposal. Administer exactly as directed. Contact prescriber if there is breast enlargement in male, difficulty in breathing, bleeding from nose, acute stomach disturbance, pain at injection site, fever or swelling/warmth/redness/pain in calves.
