Indications
Moderately severe to severe Alzheimer's dementia.
Dosage
Adults: Initially 5 mg daily during 1st wk increasing at wkly intervals in increments of 5 mg daily to maintenance of 10 mg twice daily by 4th wk. Children: Not recommended.
Adverse Effects
Hallucinations, confusion, dizziness, headache, tiredness, hypertension, constipation, somnolence, dyspnoea, elevated LFTs, balance disorders.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Severe hepatic impairment. Precautions: Moderate renal, hepatic impairment, epilepsy or predisposition to epilepsy, history of convulsions, recent MI, uncompensated heart failure, uncontrolled hypertension.
Interactions
Amantadine, ketamine, dextromethorphan, levodopa, dopaminergic agonists, anticholinergics, barbiturates, neuroleptics, baclofen, dantrolene, cimetidine, ranitidine, procainamide, quinidine, quinine, nicotine, hydrochlorothiazide, antipsychotics, oral anticoagulants, phenytoin.
Advice to Patient
Take with or without meals. Avoid a diet excessively high in fruits and vegetables.
Pharmacokinetics
Absorption: Well absorbed. Metabolism: Partially hepatic. Half-life elimination: Terminal: Approx. 60 to 80 hours. Time to Peak, serum: Immediate release: 3 to 7 hours; Extended release: 9 to 12 hours. Excretion: Urine (74%; approx. 48% of the total dose as unchanged drug; undergoes active tubular secretion moderated by pH-dependent tubular reabsorption; excretion reduced by alkaline urine pH).
