Antidepressant · Tricyclic antidepressant (TCAD)
Indications
Depression.
Dosage
Adults: Initially 25-75 mg daily in one dose (usually at night) or three divided doses, adjusting after 1-2 wk as necessary. Max 150 mg daily. Elderly: Initially 30 mg at night or 10 mg three times daily. Children: Not recommended.
Adverse Effects
Convulsions, skin rash, impaired reactions, anticholinergic effects.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Mania, severe liver or renal disease, history of epilepsy, narrow angle glaucoma, urinary retention, recent MI. Precautions: Cardiovascular diseases, elderly.
Interactions
MAOIs, antihypertensives, sympathomimetics, barbiturates, antipsychotics, alcohol, anaesthetics.
Advice to Patient
Take with meals. Do not stop taking medication abruptly, gradually lower the dose and then stop. Contact prescriber if there is difficulty in urination, increased drowsiness, fever or sore throat. Do not take other CNS depressants during therapy. Avoid alcohol intake during therapy. On start of therapy or change of dosage, patient should be watched carefully for suicidal tendency by caregiver or family.

Absorption: Slowly but completely absorbed from the GI tract. Metabolism: Hepatic. Bioavailability: Absolute: 66%-70% Half-life elimination: 43 hrs. Time to peak, plasma: Oral: 8 hrs. Excretion: Urine (2/3rd of dose). Faeces (1/3rd of dose).
