Lurasidone

Tranquilizer, Sedative/Hypnotic · Atypical antipsychotic

Indications

Schizophrenia, bipolar depression.

Dosage

Schizophrenia: ORAL: Adults: 40 mg once daily, increased if necessary up to 160 mg daily. Children: 13-18 yrs: 40 mg once daily, increased if necessary up to 80 mg once daily. Bipolar depression (monotherapy or as adjunct to lithium or divalproex): ORAL: Adults: 20 mg once daily, increased if necessary up to 120 mg once daily. Children: 10-18 yrs: 20 mg once daily as monotherapy, increased after one wk if necessary up to 80 mg once daily.

Adverse Effects

Anxiety, musculoskeletal stiffness, catatonia, reduced appetite, dysarthria, dysuria, hot flush, myalgia, nightmares.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Patients with dementia-related psychosis. Precautions: Elderly. Moderate and severe renal impairment. Moderate and severe hepatic impairment.

Interactions

Strong CYP3A4 inhibitors (avoid) (e.g. ketoconazole, clarithromycin, ritonavir, voriconazole, mibefradil). Strong CYP3A4 inducers (avoid) (rifampin, avasimibe, St. John’s wort, phenytoin, carbamazepine).

Advice to Patient

Take with meals. Avoid grapefruit and grapefruit juice during treatment. Avoid driving, operating machinery and doing tasks that require alertness. Keep yourself hydrated by drinking plenty of water, staying in a cool place and avoiding heavy clothing. Contact prescriber immediately if there is high fever, stiff muscles, confusion, increased sweating, difficulty in breathing or change in BP/heart rate.

Absorption: Increased in fed state. Metabolism: Hepatic. Bioavailability: 9%-19%. Half-life elimination: 18-40 hrs. Time to peak: 1-3 hrs; steady state concentrations achieved within 7 days. Excretion: Urine (approx. 9%). Faeces (approx. 80%).
