Tranquilizer, Sedative/Hypnotic · Atypical antipsychotic

Indications

Schizophrenia in adults. Depressive episodes associated with bipolar I or II disorder (bipolar depression) in adults, as monotherapy and as adjunctive therapy with lithium or valproate.

Dosage

Adults: 42 mg once daily. Children: Not recommended.

Adverse Effects

Nausea, dry mouth, dizziness, somnolence/sedation.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Caution advised or effect undetermined. Contraindications: Dementia-related psychosis. Precautions: Moderate or severe hepatic impairment. History of leukopaenia or neutropaenia; perform CBC in patients with pre-existing low WBC count. Known cardiovascular or cerebrovascular disease. History of seizure. Conditions that lower seizure threshold.

Interactions

CYP3A4 inducers e.g. Rifampin; avoid. Strong and moderate CYP3A4 inhibitors e.g. Itraconazole and Diltiazem; reduce dose.

Advice to Patient

Take capsule with or without meals. Avoid operating machinery and doing tasks that require mental alertness during treatment. Males and females of reproductive potential must use effective contraception during treatment. Do not become too hot or dehydrated during treatment. Do not wear heavy clothing. Stay out of the sun. Drink plenty of water.
