Loteprednol

Corticosteroid, Eye · Anti-inflammatory

Indications

Post-operative inflammation.

Dosage

Adults: 1-2 drops four times daily starting 24 hr after surgery; max treatment duration 2 wk. Children: Not recommended.

Adverse Effects

Discomfort, itching, dry eye, foreign body sensation, epiphora, corneal defect, conjunctival hyperaemia, instillation site burning, headache.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Viral, mycobacterial, fungal, amoebic or purulent infections of cornea and conjunctiva. Undiagnosed red eye. Hypersensitivity to other corticosteroids. Precautions: Prolonged use. Glaucoma. Monitor lOP when used for 10 days or longer, Soft contact lenses; wait minimum, 15 min before inserting. Risk of visual disturbances.

Interactions

Ocular hypotensive agents, cycloplegics.

Advice to Patient

Remove contact lens and reinsert atleast 10 minutes after instillation. To instill medication into eyes, sit down then tilt head back and place medication in conjunctival sac and close eyes. Roll eyeball in all directions. Close eye and apply little pressure over inside corner of the eye for 1 minute. Contact prescriber if you experience changes in vision, swelling in eyelids, extreme itching, constant inflammation or if there is no improvement. Avoid prolonged use.

Pharmacokinetics

Onset: = 2 hrs. Absorption: Well-absorbed into the eye. Limited systemic absorption. Half-life elimination: 2.8 hrs.
