Indications

Lower respiratory infection, acute bacterial exacerbation of chronic bronchitis, urinary tract infection caused by sensitive pathogens.

Dosage

Lower Respiratory Infection: Adults: >18 yr: 400 mg once daily x 10 days. Urinary Tract Infection: Adults: >18 yr: Complicated UTI: 400 mg once daily for 14 days. Uncomplicated Cystitis: 400 mg once daily for 3 days (E. coli) or 10 days (K. pneumoniae, P. mirabilis, or S. saprophyticus). Children: Not recommended.

Adverse Effects

Headache, dizziness, photosensitivity, nausea.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Hypersensitivity to quinolones. Precautions: CNS stimulation occur. Caution in patients with psychosis or a history of psychiatric disease. Risk of seizures. Avoid exposure to strong UV light.

Interactions

Antineoplastic agents, oral anticoagulants, NSAIDs, CYP1A2 substrates, loop diuretics, probenecid, metoclopramide, corticosteroids, methotrexate, magnesium, aluminium, calcium or iron salts, theophylline and other xanthine derivatives.

Advice to Patient

Take with or without meals, with a full glass of water. Take atleast 4 hours before or 8 hours after antacids or products containing calcium, zinc or iron. Inform prescriber and discontinue medication if there are signs of inflammation or tendon pain or if there is rash, itching, sunburn, redness, swelling, difficulty in breathing, facial edema, palpitations, tingling or chest pain. Contact prescriber if there is chest pain, palpitations, seizures, severe persistent constipation or diarrhoea, unusual chills or fever, vaginal itching, foul-smelling vaginal discharge, easy bleeding or bruising. Complete full course of therapy even if you feel better. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Absorption: Oral: Rapidly and almost completely absorbed from the GI tract. Metabolism: Undergoes limited metabolism. Half-life elimination: 7-8 hrs. Prolonged in patients with renal impairment. Time to peak, plasma: 1.5 hrs. Excretion: Urine (75%). Faeces (10%).
