Linagliptin
Hypoglycaemic excluding Insulin · Dipeptidyl peptidase-4 inhibitor
Indications
Type II diabetes inadequately controlled by diet and exercise: as monotherapy when metformin alone is inadequate; with metformin when metformin alone is inadequate, with metformin and a sulfonylurea when dual therapy is inadequate; or with insulin when insulin with or without metformin, is inadequate.
Dosage
Adults: 5 mg daily. In combination therapy consider lower dose of sulfonylurea or insulin. Children: Not recommended.
Adverse Effects
Hypoglycaemia.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Diabetic ketoacidosis. Precautions: Risk of pancreatitis; advise patient of symptoms, discontinue if suspected. Elderly > 80 years.
Interactions
Potent CYP3A4 and P-gp inducers.
Advice to Patient
Take with or without meals.
Pharmacokinetics
Metabolism: Via minor elimination pathway. A small fraction is converted into an inactive metabolite. Bioavailability: Approx 30%. Time to peak, plasma: 1.5 hrs. Excretion: Faeces (80%). Urine (5%).
