Irbesartan and Amlodipine

Angiotensin-II Antagonist · Angiotensin II antagonist/Calcium antagonist

Indications

Treatment of essential hypertension. Patients whose blood pressure is not adequately controlled on Irbesartan or Amlodipine monotherapy.

Dosage

Adults: 1 tab daily. Max: 300 mg/10 mg daily. Children: Not recommended.

Adverse Effects

Headache, drowsiness, dizziness, fatigue, oedema, nausea, visual disturbances, abdominal pain, flushing, dyspnoea, dyspepsia, altered bowel habits.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Shock (including cardiogenic shock), obstruction of the left ventricular outflow tract (e.g. clinically significant aortic stenosis), unstable angina (excluding Prinzmetal angina), hemodynamically unstable heart failure following an acute myocardial infarction, severe hypotension. Precautions: Hepatic impairment. Psoriasis. Hypertensive patients with stenosis of the renal artery of one or both kidneys, or patients with severe congestive heart failure. Elderly.

Interactions

Aliskiren in patients with diabetes or with moderate to severe renal impairment. ACE inhibitors.

Advice to Patient

Take with or without meals. Be cautious while driving or doing tasks that require alertness e.g. operating machinery.

IRBESARTAN: Onset of action: Peak effect: 1-2 hrs. Max effect: 3-6 hrs postdose; with chronic dosing max effect: Approx. 2 wks. Duration: >24 hrs. Metabolism: Hepatic. Bioavailability: 60%-80%. Half-life elimination: Terminal: 11-15 hrs. Time to peak, serum: 1.5-2 hrs. Excretion: Faeces (80%). Urine (20%). AMLODIPINE: Onset of action: Antihypertensive effect: Significant reductions in blood pressure at 24-48 hrs after first dose. Duration: Antihypertensive effect: At least 24 hrs. Discontinuation after 6-7 wks of treatment: Duration extends to at least 72 hrs. Absorption: Well absorbed. Metabolism: Hepatic. Bioavailability: 64%-90%. Half-life elimination: Terminal (biphasic): 30-50 hrs. Time to peak, plasma: 6-12 hrs. Excretion: Urine.
