Iloperidone

Tranquilizer, Sedative/Hypnotic · Atypical antipsychotic

Indications

Acute treatment of schizophrenia.

Dosage

Adults: Initial: 1 mg two times daily. Titrate: 2 mg two times daily on day 2, 4 mg two times daily on day 3, 6 mg two times daily on day 4, 8 mg two times daily on day 5, 10 mg two times daily on day 6, 12 mg two times daily on day 7. Range: 6-12 mg two times daily. Max: 12 mg two times daily (24 mg/day). Children: Not recommended.

Adverse Effects

Dizziness, dry mouth, fatigue, nasal congestion, orthostatic hypotension, somnolence, tachycardia and weight increased. Priapism, leukopenia, neutropenia, agranulocytosis.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Dementia-related psychosis. Precautions: Elderly patients with dementia-related psychosis. Avoid use in combination with other drugs that are known to prolong QTc. Neuroleptic malignant syndrome, Tardive dyskinesia. Use cautiously in patients with a history of seizures, use caution when operating machinery.

Interactions

The dose of Iloperidone should be reduced in patients co-administered with a strong CYP2D6 or CYP3A4 inhibitor.

Advice to Patient

Contact prescriber if there is change in mental status e.g. abnormal thinking. Avoid alcohol during therapy. Change position slowly from sitting or lying to standing. Avoid activities that increase body temperature, such as doing strenuous exercise, being exposed to extreme heat. If you have diabetes, regularly check blood glucose level.

Absorption: Well absorbed. Metabolism: Hepatic. Half-life elimination: Extensive metabolizers: Iloperidone: 18 hrs; P88: 26 hrs. Poor metabolizers: Iloperidone: 33 hrs; P88: 37 hrs; P95: 31 hrs. Time to peak, plasma: 2-4 hrs. Excretion: Active metabolites (P88 and P95): Urine (58% extensive metabolizers, 45% poor metabolizers). Faeces (20% extensive metabolizers, 22% poor metabolizers). Iloperidone: <1 % recovered unchanged in urine and faeces.
