Indications

Rheumatoid arthritis, juvenile rheumatoid arthritis (JRA), ankylosing spondylitis, osteoarthrosis, seronegative arthropathies, peri-articular disorders, soft tissue injuries. Mild to moderate pain. Fever.

Dosage

ORAL: Adults: 1200-1800 mg daily in divided doses. Max 2400 mg daily. Dysmenorrhoea: 400 mg every 4 hr if needed. Inflammatory Disease: 400-800 mg every 6-8 hr. Max: 3.2 gm/day. Pain/fever: 200-400 mg every 4-6 hr. Max: 1.2 gm/day. Children: Not recommended for children under 3 mth or body-weight under 5 kg. Administration

Administer for at least 30 mins in adult patients and 10 mins in paediatric patients. Patient must be well hydrated prior to administration. Vials must be diluted prior to administration. Reconstitution: Dilute to a final conc of 4 mg/mL or less. 100 mg dose: Dilute 1 mL of vial in at least 100 mL of diluent. 200 mg dose: Dilute 2 mL of vial in at least 100 mL of diluent. 400 mg dose: Dilute 4 mL of vial in at least 100 mL of diluent. 800 mg dose: Dilute 8 mL of vial in at least 200 mL of diluent. Compatibility: Compatible diluents include NS, D5W, or LR. Stability: Store at controlled room temp 20°C-25°C; excursions permitted between 15°C-30°C.

Adverse Effects

Gl upset or bleeding, rash, rarely thrombocytopenia, rarely aseptic meningitis, neutropenia, periventricular leukomalacia, bronchopulmonary dysplasia, necrotising enterocolitis, intestinal perforation, oliguria, fluid retention, haematuria. Raised creatinine and hyponatraemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Severe renal impairment. Life threatening infection, active bleeding, thrombocytopenia or coagulation defects, necrotising enterocolitis, marked hyperbilirubinaemia, ductal-dependent congenital heart disease. Precautions: Monitor renal and GI function. ECG must be carried out before administration to exclude pulmonary hypertension and ductal-dependent congenital heart disease. If hypoxaemia occurs, monitor pulmonary pressure. Infection.

Interactions

Anticoagulants, thiazide diuretics, quinolones, aminoglycosides, nitric oxide, antihypertensives, digoxin, lithium, methotrexate, cyclosporin, mifepristone, other NSAIDs, corticosteroids.

Advice to Patient

Take immediately after meals. Take tablets with a glass of water. Do not lie down for 15-30 minutes after taking medication. Do not take medication for more than 3 days for fever and more than 10 days for pain without consulting prescriber. Discontinue taking medication and inform prescriber if there is unrelieved nausea or vomiting, constant stomach pain or cramps, indigestion, ringing in ears, changes in hearing, unusual bruising or bleeding, difficulty in breathing, blood in urine/mouth/stool, unusual swelling of extremities, skin rash, itching, blisters, chest pain, palpitations, difficulty in speaking. Avoid alcohol, increased amount of vitamin C, salicylate containing foods (curry powder, prunes, raisins, tea, licorice), medications containing aspirin or salicylate, NSAIDS during therapy without consulting prescriber.
