Erythromycin and Sulfisoxazole

Indications

Treatment of bacterial infections of upper and lower respiratory tract, otitis media in children caused by H. influenzae, infections in patients allergic to penicillins.

Dosage

Children: >2 mth: Dose based on 50 mg/kg/day erythromycin or 150 mg/kg/day sulfisoxazole given in 3-4 divided doses for 10 days. Max: 6 gm/day sulfisoxazole. Adults: >45 kg 400 mg erythromycin and 1200 mg sulfisoxazole every 6 hr.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended.

Advice to Patient

Take with or without meals. If there is stomach upset, take with meals. Take at the same time each day. Take plenty of fluids. If a dose is missed, take it as soon as you remember unless it is almost time for the next dose. Do not take double dose. Complete full course of therapy even if you feel better before completion of course. Avoid grapefruit juice during therapy.

ERYTHROMYCIN: Absorption: Oral: Variable but better with salt forms than with base form; 18%-45%; ethylsuccinate may be better absorbed with food. Metabolism: Hepatic. Half-life elimination: Neonates (=15 days of age): 2.1 hrs. Adults: Peak: 1.5-2 hrs. End-stage renal disease: 5-6 hrs. Time to peak, serum: Base: 4 hrs. Ethylsuccinate: 0.5-2.5 hrs. Stearate: 3 hrs; delayed with food due to differences in absorption. Excretion: Primarily faeces. Urine. SULFISOXAZOLE: Absorption: Readily absorbed from the GI tract. Metabolism: Digestive enzymes in the GI tract convert acetyl sulfisoxazole to sulfisoxazole. Half-life elimination: 10-12 hrs. Time to peak, plasma: 2-6 hrs. Excretion: Urine (97%).
