Laxative · Bile acid transporter inhibitor

Indications

Chronic constipation including chronic idiopathic constipation, except for constipation associated with organic diseases.

Dosage

Adults: 10 mg orally once daily before meal. Max: 15 mg/day. Children: Not recommended.

Adverse Effects

Abdominal pain/discomfort, diarrhoea, loose stools, flatulence, nausea, LFT abnormalities (resolve upon discontinuation).

Risk Factor (Pregnancy/Lactation)

Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Contraindicated or not recommended. Contraindications: Known or suspected intestinal obstruction (e.g., tumors, hernias) or conditions where bowel obstruction could be aggravated. Precautions: Elderly. Severe hepatic impairment. Underlying GI diseases.

Interactions

Aluminum-containing antacids, sucralfate hydrate, aldioxa, bile acid sequestrants/absorbents (e.g. cholestyramine, colestimide), P-glycoprotein substrates (e.g. digoxin, dabigatran etexilate, midazolam).

Advice to Patient

Take tab before meals ideally once daily before breakfast. Report severe diarrhea or stomach pain. Do not take OTC laxatives concurrently without consulting prescriber.

Pharmacokinetics

Absorption: Food reduces systemic exposure but promotes the intended intraluminal bile acid action. Bioavailability: Very low. Metabolism: Hepatic (partially). Excretion: Faeces (primarily). Urine (minimal).
