Indications
Dysmenorrhoea, habitual and threatened abortion, endometriosis, infertility due to inadequate luteal phase. Premenstrual syndrome. Adjunct to oestrogen in replacement therapy.
Dosage
Adults: Dysmenorrhoea: 1 tab twice daily from day 5 to day 25 of the cycle. Endometriosis: 1 tab two or three times daily from day 5 to day 25 of the cycle or continuously. Premenstrual syndrome: 1 tab twice daily from day 12 to day 26 of the cycle. Hormone replacement: 2 tab daily for 12 days each mth. Children: Not applicable.
Adverse Effects
Menstrual disturbances, premenstrual-like syndrome (including bloating, fluid retention, breast tenderness), weight change, nausea, headache, dizziness, insomnia, drowsiness, depression, change in libido; also skin reactions (including urticaria, pruritus, rash and acne), hirsutism and alopecia. Jaundice and anaphylactoid reactions have also been reported.
Risk Factor (Pregnancy/Lactation)
Contraindications: Progestogens should be avoided in patients with a history of liver tumours, with genital or breast cancer (unless progestogen are being used in the management of these conditions), severe arterial disease, undiagnosed vaginal bleeding and acute porphyria. Progestogens should not be used if there is a history during pregnancy of idiopathic jaundice, severe pruritus, or pemphigoid gestations. Precautions: Progestogens should be used with caution in conditions that may worsen with fluid retention e.g. epilepsy, hypertension, migraine, asthma, or cardiac dysfunction and in those susceptible to thromboembolism (particular caution with high dose, history of depression. Progestogens can decrease glucose tolerance and patients with diabetes should be monitored closely.
Interactions
Rifamycin, anticoagulants, antidepressants, antidiabetics, anti-epileptics, antifungals, antivirals, aprepitant, barbiturates, bosentan, cyclosporin, diuretics, dopaminergics, lipid-regulating drugs, muscle relaxants.
Pharmacokinetics
Absorption: Rapid. Metabolism: Completely metabolized to 20-dihydrodydrogesterone (DHD) metabolite. Bioavailability: 28%. Half¬-life elimination: Dydrogesterone: 5-7 hrs. 20-dihydrodydrogesterone (DHD): 14-17 hrs. Time to peak, plasma: 0.5-2.5 hrs. Excretion: Urine (63%).
