Indications

Complicated intra-abdominal infections. Complicated UTIs including pyelonephritis.

Dosage

IV: Adults: Intra-abdominal infection, complicated: 500 mg every 8 hrs for 5-14 days. Urinary tract infection, complicated (including pyelonephritis): 500 mg every 8 hrs for 10 days (14 days with concurrent bacteraemia). Urinary tract infection (complicated) or pyelonephritis: 500 mg every 8 hrs for 10-14 days.

Administration

IV: Infuse over 1 hr. Reconstitution: Reconstitute 250 mg and 500 mg vial with 10 mL of SWFI or NS, further dilute the 250 mg vial for infusion with 50 mL or 100 mL of NS or D5W. Further dilute the 500 mg vials for infusion with 100 mL of NS or D5W. Shake gently until clear. To prepare a 250 mg dose using a 500 mg vial, reconstitute the 500 mg vial and further dilute with 100 mL of NS or D5W as above: remove and discard 55 mL from the infusion bag (remaining soln contains 250 mg). Colour may vary from colourless to slightly yellow. Compatibility: Stable in SWFI, NS and D5W. Stability: Store intact vials at 25°C; excursions permitted to 15°C-30°C. Reconstituted vial may be stored for up to 1 hr prior to preparation of infusion soln. Stability of soln when diluted in NS is 12 hrs at room temp or 72 hrs at 2°C-8°C; stability in D5W is 4 hrs at room temp and 24 hrs at 2°C-8°C. Do not freeze.

Adverse Effects

Headache, phlebitis, pruritus, skin rash, C. difficile associated diarrhoea, nausea, oral candidiasis, anaemia, vaginal infection, increased serum transaminases. Rare: anaphylaxis, neutropaenia, leukopaenia, thrombocytopaenia, Stevens-Johnson syndrome.

Risk Factor (Pregnancy/Lactation)

Contraindications: Patients with known serious hypersensitivity to doripenem, patients with known serious hypersensitivity reactions to other carbapenems, in patients who have demonstrated anaphylactic reactions to beta-lactam antibiotics. Precautions: Renal impairment. CNS disorders (e.g. brain lesions, stroke, or history of seizures).

Interactions

BCG vaccine (intravesical), cholera vaccine, typhoid vaccine, lactobacillus and estriol, sodium picosulphate, probenecid, valproate products.

Pharmacokinetics

Metabolism: Non-CYP-mediated metabolism via hydrolysis. Half-life elimination: Approx. 1 hr. Excretion: Urine (86%). Faeces (<1%).
