Indications

Gastrointestinal disorders characterised by smooth muscle spasm for relief of flatulence, abdominal discomfort, infant colic (Wind) due to entrapped gas.

Dosage

TABLET: Adults: 10-20 mg three times daily. SYRUP: Infants & Children: 6 mth-2 yr, 2.5-5 mL before each feed, max four doses in 24 hr. 2-12 yr, 5 mL three times daily. 12-18 yr, 5-10 mL three times daily. Adults: 10 mL three to four times daily.

Adverse Effects

Dry mouth, thirst and dizziness.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Infants under 6 months. Precautions: Patients with actual or suspected glaucoma or prostatic hypertrophy. May exacerbate acid reflux in hiatus hernia.

Interactions

Amantadine, atenolol, digoxin, phenothiazines, tricyclic antidepressants.

Advice to Patient

Take 30 to 60 minutes before meals. Do not take antacid or an anti-diarrhoeal within 2 hours of taking dicyclomine. Take high-fiber foods and drink at least 8 glasses of water daily to prevent constipation. Avoid extreme heat and humidity. Change position slowly to avoid light-headedness. Do not stop taking the drug abruptly, instead stop gradually to avoid dizziness and vomiting.

Pharmacokinetics

Absorption: Rapid. 67% of an oral dose is absorbed from the GI tract. Bioavailability: For IM inj, it is greater than that of oral dose. Half-life elimination: For the first 9 hrs after a single dose: 1.8 hrs. Time to peak, plasma: 1-1.5 hr. Excretion: Urine (79.5%). Faeces (8.4%).
