Indications
Increase tear production when suppressed tear production is presumed to be due to keratoconjunctivitis sicca-associated ocular inflammation (in patients not already using topical anti-inflammatory drugs or punctual plugs).
Dosage
Keratoconjunctivitis sicca: Children = 16 yr and Adults: Instill 1 drop in each eye every 12 hr.
Adverse Effects
Abnormal vision, cataract, conjunctivitis, eye pain, visual disturbance. Hyperlipidaemia, hyperuricaemia, hyperkalaemia, hypomagnesaemia, tremor, headache, paraesthesia, hypertension (discontinue if cannot control), GI upset, anorexia, abdominal pain, gingival hypertrophy, hepatic and renal dysfunction, hypertrichosis, muscle cramps, myalgia, fatigue, anaemia, thrombocytopenia, signs of encephalopathy, rash, oedema, weight gain, malignancies, Iymphoproliferative disorders, infections. Rarely, micro-angiopathic haemolytic anaemia or haemolytic uraemic syndrome, menstrual disturbances, gynaecomastia, hyperglycaemia, motor polyneuropathy, pancreatitis, muscle weakness, myopathy, optic disc oedema. Risk of benign intracranial hypertension; investigate patients presenting with signs of raised intracranial pressure, withdraw cyclosporin if diagnosed.
Risk Factor (Pregnancy/Lactation)
Contraindications: Active ocular infections. Precautions: Safety and efficacy have not been established in patients < 16 years of age.

Tacrolimus, other immunosuppressants, barbiturates, carbamazepine, oxcarbazepine, primidone, phenytoin, griseofulvin, trimethoprim, rifampicin, octreotide, orlistat, St. John's Wort, macrolides, azole antifungals, calcium channel blockers, metoclopramide, oral contraceptives, danazol, allopurinol, amiodarone, ursodeoxycholic acid, protease inhibitors, imatinib, nephrotoxic drugs, NSAIDs, vaccines, digoxin, corticosteroids, statins, etoposide, colchicine, K+-sparing diuretics, K+ supplement, ACE inhibitors, angiotensin II antagonists, grapefruit juice, methotrexate, H2 blockers, propafenone, bromocriptine, doxycycline, chloroquine, sulfinpyrazone, terbinafine, bosentan, substrates of CYP3A4 and/or P-glycoprotein, repaglinide.
