Indications
Pulmonary infection due to P. aeruginosa in cystic fibrosis.
Dosage
Adults and children: Under 2 years: 0.5-1 MIU twice daily. Over 2 years: 1-2 MIU two or three times daily.
Adverse Effects
Neurotoxicity, confusion, visual disturbances, nephrotoxicity, hypersensitivity reactions, bronchospasm, pruritus, skin rash, urticaria, apnoea, fever, respiratory distress.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Caution advised or effect undetermined. Contraindications: Myasthenia gravis. Precautions: Renal impairment. Porphyria, haemoptysis.
Interactions
Cephalosporins, aminoglycosides, other drugs with nephrotoxic or neurotoxic potential, neuromuscular blockers.
Pharmacokinetics
Absorption: Not absorbed from the GI tract, mucous membranes, or intact skin. Metabolism: Hydrolyzed to colistin (active form). Half-life elimination: IM, IV: Colistimethate: 2-3 hrs. Critically ill: Infants, Children, Adolescents and Adults: IV: Colistimethate: 2.3 hrs; Colistin: 14.4 hrs. Cystic fibrosis: IV: Colistin: Approx. 4 hrs. ESRD patients receiving CAPD: IV: Colistin: 13.2 hrs. Time to peak: Healthy volunteers: IV: Colistin: 2 hrs. Critically ill: IV: Colistin: Approx. 7 hrs. Excretion: Primarily urine.
