Indications

Symptoms and signs of cerebral insufficiency (mental deterioration) like dizziness, headache, poor concentration, disorientation, impaired memory, lack of initiative, mood depression, unsociability, difficulties with daily living activities and with self-care. Acute cerebrovascular diseases. Migraine and vascular headaches (preventive treatment only). Dizziness and vertigo of different origin.

Dosage

Adults: ORAL: 1 tab (1.5 mg) or 30 drops 2 to 3 times daily preferably before meals or 2 to 3 tab or 60-90 drops or 1 tab of 4.5 mg once daily before breakfast for 3 mth or more. Children: Not recommended. Adults: INJECTION: In dizziness, tinnitus and vertigo: The therapy should preferably be started with 1 amp (1 mL) twice daily intramuscularly for one wk, followed by therapy with tab. In acute cerebrovascular disorders and cerebrovascular accident (stroke): Should be administered according to the following schedule: Day 1 to day 3, 9-10 mL in 1000 mL of physiological saline given as an IV drip, every 24 hr. Day 4 to day 10, 3 mL IM every 12 hr. Followed by oral therapy with tab/soln. Children: Not recommended.

Administration

Doses of 300 mcg can be given IM, SC or by IV infusion.

Adverse Effects

Gl upset, flushes, rash, nasal congestion, cramps, headache, dizziness.

Risk Factor (Pregnancy/Lactation)

Precautions: Severe bradycardia.

Advice to Patient

Take just before meals. Do not chew or crush tablets, swallow whole with a glass of water. Contact prescriber if there is dizziness, lightheadedness, stomach upset or stomach cramps, hot/cold flushes, skin rash, nasal stuffiness, headaches and dizziness. Avoid alcohol during therapy.

Absorption: Rapidly absorbed from the GI tract. Metabolism: Hepatic. Bioavailability: Oral: Low (25%) due to incomplete absorption from the GI tract and extensive first-pass metabolism. Half-life elimination: Biphasic. Alpha phase: 1.5-2.5 hrs. Beta phase: 13-15 hrs. Time to peak, plasma: Oral: 1-2 hrs. Excretion: Faeces (mainly). Urine (small amounts as metabolites and unchanged drug).
