Corticosteroid Combinations, Topical · Aminoglycoside/Corticosteroid/Polyene antifungal

Indications

Psoriasis, intractable eczemas and other inflammatory dermatoses when bacterial and/or candidal infection is present or suspected.

Dosage

Adults: Apply sparingly once or twice daily; max 50 gm weekly. May be used for max four wk, then review. Children: Same as adult but review weekly.

Adverse Effects

Cushingoid changes, adrenal suppression, local skin reactions, acne, atrophy, striae, telangiectasia, mild pigmentation, hypertrichosis, pruritus, folliculitis. Hypersensitivity reactions incl contact dermatitis (discontinue).

Risk Factor (Pregnancy/Lactation)

Contraindications: Untreated bacterial, fungal or viral infections or parasitic infestations, use on burns, acne or rosacea, perioral dermatitis, widespread plaque psoriasis, perianal or genital pruritus, pruritus without inflammation. Precautions: Do not use potent steroids to treat diaper dermatoses in infants. Avoid extensive or prolonged use (limit to max 7 days in children). If using potent steroids in children or on face, limit to 5 days and avoid occlusion. Avoid mucous membranes and eye area. Broken, inflamed or occluded skin, psoriasis, dermatitis around chronic leg ulcers. Risk of visual disturbances. Elderly.

Advice to Patient

Wash and dry the skin before applying. Apply a thin film to cover affected areas and rub gently. Wash hands thoroughly after applying, unless hands are the area being treated. Measure how much amount of cream is to be used with your fingertips. For adults: Two fingertips of cream will cover both hands or one foot, three fingertips of cream will cover one arm, six fingertips of cream will cover one leg, fourteen fingertips of cream will cover the front and back of the body. For children: Not recommended for children < 2 yrs. A child of 4 years needs about one-third of the adult amount. If you have thick patches of psoriasis on elbows or knees, cream may be applied under an airtight dressing at night as recommended by the prescriber. After sometime, apply as per normal schedule. CLOBETASOL: Absorption: It ranges from about 1% in areas with a thick stratum corneum (such as the palms, soles, elbows, and knees) to as high as 36% in areas with a thin stratum corneum (face, eyelids, and genitals). Absorption increases in areas of skin damage, inflammation, or occlusion. Some systemic absorption of topical corticosteroids occurs, especially through the oral mucosa. Metabolism: After topical administration, drug is metabolized primarily in the skin. The small amount absorbed into systemic circulation is metabolized hepatically. Excretion: Urine (inactive metabolites). Faeces (small amounts of the metabolites). NEOMYCIN: Absorption: Not absorbed through intact skin. It may be absorbed from wounds, burns, or skin ulcers. Metabolism: Not metabolized. NYSTATIN: Absorption: Not absorbed through the skin or mucous membranes. Excretion: Faeces.
