Indications
Infections of the lower respiratory tract, skin and soft tissues, complicated upper and lower UT infections, neutropenia, bacteraemia, septicaemia.
Dosage
Adults: 1-2 gm every 12 hr by IV bolus over 3-5 min or by IV infusion over 20-30 min. Children: Not recommended.
Administration
By IV Injection: Dissolve the content of cefpirome 1 g vial in 10 mL sterile water for injection. Inject slowly over 3 to 5 min. By Short IV infusion: Dissolve the content of 1 g in 100 mL sterile water for injection and infuse over 20 to 30 min. Effervescence occurs on dissolution. Compatibility: NaCl 0.9%, dextrose 5% plus NaCl 0.9%. Stability: Reconstituted solution can be stored for up to 6 hours at room temperature and 24 hours in refrigerator (at 2-8° C) when prepared in water for injection BP.
Adverse Effects
Nausea, vomiting, diarrhoea, raised liver enzymes and creatinine, blood dyscrasias, injection site reactions, taste disturbance, hypersensitivity reactions. Rarely superinfections, pseudomembranous colitis, interstitial nephritis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Precautions: Renal impairment. Elderly. Hypersensitivity to beta-lactam antibiotics.
Interactions
Aminoglycosides, loop diuretics.
Pharmacokinetics
Metabolism: Not significantly metabolized. Bioavailability: IM: >90%. Half-life elimination: 2 hrs. Prolonged in patients with renal impairment. Excretion: Urine (80%-90%).
