Cariprazine
Tranquilizer, Sedative/Hypnotic · Atypical antipsychotic
Indications
Schizophrenia. Bipolar mania, bipolar depression.
Dosage
Adults: Schizophrenia and Bipolar Mania: 1.5 mg once daily. If required, increase in steps of 1.5 mg. Max: 6 mg/day. Bipolar depression: 1.5 mg once daily. If required, increase in steps of 1.5 mg. Max: 3 mg/day. Children: Not recommended.
Adverse Effects
Dizziness, drowsiness, fatigue, hypertension, akathisia, parkinsonism, dystonia, other extrapyramidal symptoms, wt and appetite changes, GI upset, dyslipidaemia, sleep disorders, anxiety, blurred vision, tachyarrhythmia, raised liver enzymes, raised creatine kinase. Reports of cataracts.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Severe hepatic or renal impairment (CrCl < 30ml/min). Ensure contraception in women of reproductive potential during and for 10 weeks after treatment (add barrier method if using hormonal contraceptives). Precautions: Monitor for akathisia. Cardiovascular disease, family history of QT prolongation, risk factors for stroke or VTE. Risk of neuroleptic malignant syndrome; discontinue if occurs. Parkinson's disease, history of seizures, risk factors for diabetes. Monitor BP and wt. Monitor high-risk patients for suicidal behaviour. Risk of tardive dyskinesia; consider discontinuation if occurs. Elderly (=65 years).
Interactions
CYP3A4 inhibitors or inducers, P-gp substrates (e.g, dabigatran, digoxin), hormonal contraceptives, other CNS drugs, drugs that prolong QT interval, alcohol, grapefruit juice.
Advice to Patient
Take with or without meals. Immediately inform prescriber if patient experiences any kind of suicidal thoughts. Be cautious while doing tasks that require mental alertness such as operating machinery and driving vehicle. Avoid exposure to heat and maintain care to prevent dehydration.
Pharmacokinetics
Onset of action: 1-2 wks. Metabolism: Hepatic. Converted to active metabolites DCAR and DDCAR. Half-life elimination: Cariprazine: 2-4 days. DCAR: 1-2 days. DDCAR: 1-3 wks. Time to peak, plasma: Cariprazine: 3-6 hrs. Excretion: Urine (21%).
