Carboplatin
Cytostatic · Alkylating agent
Indications
Ovarian tumours and also in those cases when cisplatin cannot be administered because of contraindication e.g. if renal impairment occurs.
Dosage
Advanced Ovarian Carcinoma: Adults: Single Agent: 360 mg/m² IV on day 1, then every 4 wk. Combination Therapy: 300 mg/m² IV on 1, then every 4 wk x 6 cycles. Adjust based on platelet & neutrophil count. Children: Not recommended.
Administration
Administer as IV push or bolus over 15 min up to a continuous intravenous infusion over 24 hour. Reconstitute powder to yield a final conc of 10 mg/mL. May also be administered intraperitoneally. When administered as sequential infusions, taxane derivatives (docetaxel, paclitaxel) should be administered before platinum derivatives to limit myelosuppression and to enhance efficacy. Do not use needles or IV administration sets containing aluminium parts that may come in contact with carboplatin (aluminium can react causing precipitate formation and loss of potency). Compatibility: Stable in D5¼ NS, D5½ NS, D5NS, D5W, NS. Stability: Store at 25°C (77°F); excursions permitted from 15°C-30°C (59°F-86°F). Protect from light. Adverse Effects
GI disturbances, neurologic toxicity, myelosuppression, nephrotoxicity, peripheral neuropathy, dysgeusia, hearing impairment, altered LFTs, renal impairment, electrolyte disturbances, asthenia. Reports of anaphylaxis.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Contraindicated or not recommended. Contraindications: Severe renal impairment and myelosuppression, severe allergy to platinum components or mannitol. Precautions: Peripheral blood counts, renal and neurological function to be monitored closely. Mild to moderate myelosuppression. Monitor audiometric function.
Interactions
Nephrotoxic compounds, ototoxic agents, other myelosuppressive agents, aminoglycosides, diuretics, aluminium.
Advice to Patient
This medication is administered intravenously. Contact prescriber if there is fever, chills, sore throat, chest pain, palpitations, difficulty in breathing, unusual bruising or bleeding, unusual fatigue, pain, numbness or tingling in hands/ankles/feet, change in hearing, muscle cramps, twitching occur. Take small, frequent meals to maintain good nutritional status. Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Avoid highly crowded places to prevent infection. Do not get vaccinated without consulting prescriber. Using soft toothbrush, cotton swabs, regular mouth care may help to prevent mouth sores.
Pharmacokinetics
Metabolism: Minimally hepatic to aquated and hydroxylated compounds. Half-life elimination: CrCl >60 mL/minute: Carboplatin: 2.6 to 5.9 hours (based on a dose of 300 to 500 mg/m²); platinum (from carboplatin): =5 days. Excretion: Urine.
