Indications

Anaesthesia (See under dosage).

Dosage

According to patient's physical status and nature of procedure. Local infiltration: 0.25% (up to 60 mL). Peripheral nerve block: 0.25% (max 60 mL); 0.5% (max 30 mL). Epidural block: Surgery, lumbar, 0.5% (max 20 mL); Surgery, caudal, 0.5% (max 30 mL); Children (up to 10 yr) using 0.25% solution, up to lower thoracic (T10) 0.3-0.4 mL/kg, up to mid-thoracic (T6) 0.4-0.8 mL/kg. Labour: Lumbar, 0.25-0.5% (max 12 mL); Caudal, but rarely used, 0.25-0.5% (max 20 mL). Sympathetic block: 0.25-0.5% (max 50 mL).

Administration

Administered by infiltration or by epidural, spinal, or peripheral or sympathetic nerve block as a single injection or repeat injections. Injection should be made slowly. Product containing preservatives should not be used for epidural or caudal block. Stability: Stored at controlled room temperature. Freezing should be avoided.

Adverse Effects

Cardiac arrest, hypotension, bradycardia, restlessness, anxiety, nausea, vomiting.

Risk Factor (Pregnancy/Lactation)

Contraindications: Children below 12 years not recommended. Precautions: Hepatic impairment.

Pharmacokinetics

Metabolism: Hepatic. Half-life elimination: Adults: 1.5-5.5 hrs. Neonates: 8 hrs. Excretion: Urine (mainly).
