Brinzolamide and Brimonidine Tartrate

Miotic, Antiglaucoma Preps · Alpha-2 agonist/Carbonic anhydrase inhibitor

Indications

Raised intra-ocular pressure in open-angle glaucoma and in ocular hypertension when monotherapy is inadequate.

Dosage

Adults: 1 drop in the affected eye two or three times daily. Children: <2 yrs: Not recommended.

Adverse Effects

Somnolence, dermatitis, dry throat, hypotension, vertigo, blurred vision, bad taste.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Severe hepatic or renal impairment. Precautions: Patients with low endothelial cell counts. Acute angle-closure glaucoma, severe cardiovascular disease, depression, cerebral or coronary insufficiency, Raynaud’s phenomenon, orthostatic hypotension, or Thromboangiitis obliterans.

Interactions

MAOIs, oral carbonic anhydrase inhibitors, high-dose salicylate therapy, CNS depressants, antihypertensives/cardiac glycosides, tricyclic antidepressants.

Advice to Patient

Wash hands properly before use. Shake suspension well before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops into the eye. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Apply gentle pressure to inner corner of eye for 30 seconds. Contact prescriber if there is eye pain; eye infection; redness, burning, watering, puffiness around eyes; changes in vision; swelling, itching in eyes; no improvement or worsening of condition. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication. Avoid doing activities that require mental alertness.

BRINZOLAMIDE: Absorption: Absorbed into systemic circulation following topical ocular application. Metabolism: Ophthalmic. Half-life elimination: 111 days. Excretion: Urine (60%). BRIMONIDINE TARTRATE: Absorption: Well absorbed into the eye. Also absorbed systemically. Metabolism: Cornea. Hepatic. Half-life elimination: 2-3 hrs. Time to peak, plasma: 0.5-4 hrs. Excretion: Urine (mainly).
