Brimonidine Tartrate, Eye
Miotic, Antiglaucoma Preps · Alpha-2 agonist
Indications
Monotherapy where beta-blockers are ineffective or contraindicated or as adjunct to beta-blockers, in open-angle glaucoma or ocular hypertension.
Dosage
Adults: 1 drop twice daily. Children: Not recommended.
Adverse Effects
Ocular hyperaemia, conjunctival follicles, stinging, pruritus, oedema, allergy. Dry mouth, headache, fatigue, drowsiness, dizziness, dysgeusia, local irritation, visual disturbance, upper respiratory symptoms, asthenia, GI upset.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Contraindicated or not recommended. Precautions: Severe cardiovascular disease. Depression, cerebral or coronary insufficiency. Raynaud's syndrome, orthostatic hypotension, thromboangiitis obliterans, renal or hepatic impairment. Pregnancy, lactation. Soft contact lenses; wait minimum 15 min before inserting.
Interactions
MAOIs, antidepressants affecting noradrenergic transmission, CNS depressants, antihypertensives, cardiac glycosides, sympathomimetic agonists.
Advice to Patient
Wash hands properly before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops into the eye. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Apply gentle pressure to inner corner of eye for 30 seconds. Contact prescriber if there is eye pain; eye infection; redness, burning, watering, puffiness around eyes; changes in vision; swelling, itching in eyes; no improvement or worsening of condition. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication. Avoid doing activities that require mental alertness.
Pharmacokinetics
Absorption: Well absorbed into the eye. Also absorbed systemically. Metabolism: Cornea. Hepatic. Half-life elimination: 2-3 hrs. Time to peak, plasma: 0.5-4 hrs. Excretion: Urine (mainly).
