Brimonidine Tartrate and Timolol, Eye
Miotic, Antiglaucoma Preps · Alpha-2 agonist/Beta-blocker
Indications
Reduction of IOP in open-angle glaucoma or ocular hypertension insufficiently responding to beta-blockers.
Dosage
Adults: 1 drop twice daily. Children: Not recommended.
Adverse Effects
Conjunctival hyperaemia, burning sensation, conjunctival folliculosis, stinging, pruritus, oedema, allergy. Allergic conjunctivitis/blepharitis; discontinue if occur. Depression, somnolence. Headache, hypertension, dry mouth, asthaenia, abnormal LFTs. Ocular irritation, keratitis, corneal erosion, visual disturbance, systemic beta blocker effects.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Contraindicated or not recommended. Contraindications: Asthma or history of asthma, severe COPD. Sinus bradycardia, sick sinus syndrome, sino-atrial block, 2nd or 3rd degree AV block not controlled with a pacemaker. Cardiac failure, cardiogenic shock. Precautions: Severe cardiovascular disease; monitor cardiac function. Hyperthyroidism, unstable angina. Severe peripheral or central circulatory disorders. Hypotension, diabetes, metabolic acidosis, pheochromocytoma, hyperthyroidism. Depression. Cerebral or coronary insufficiency, Raynaud's syndrome. Orthostatic hypotension, thromboangiitis obliterans. Renal or hepatic impairment, dialysis patients. Withdraw gradually. Soft contact lenses; wait minimum 15 min before inserting. Mild to moderate COPD, corneal disease.
Interactions
MAOIs, antidepressants affecting noradrenergic transmission, CNS depressing depressants. Antihypertensives, cardiac glycosides, sympathomimetic agonists or antagonists, general anaesthetics. IV lidocaine, iodine contrast media, cimetidine, hydralazine, TCADs, mianserin, antiarrhythmics, CYP2D6 inhibitors, alcohol, parasympathomimetics.
Advice to Patient
Wash hands properly before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops into the eye. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Apply gentle pressure to inner corner of eye for 30 seconds. Contact prescriber if there is eye pain; eye infection; redness, burning, watering, puffiness around eyes; changes in vision; swelling, itching in eyes; no improvement or worsening of condition. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication. Avoid doing activities that require mental alertness.

Excretion: Urine (mainly). TIMOLOL: Onset: 30 mins. Duration: 12-24 hrs. Absorption: Significant systemic absorption when administered to the eye. Metabolism: Hepatic. Half-life elimination: 4 hrs. Time to peak: 1-2 hrs. Excretion: Urine (mainly).
