Bimatoprost

Indications
As monotherapy or as an adjunct to beta-blockers in chronic open-angle glaucoma or ocular hypertension.
Dosage
Adults: 1 drop in affected eye(s) once daily in the evening. Children: Not recommended.
Adverse Effects
Growth or darkening of eyelashes, conjunctival hyperaemia, ocular pruritus, pain, irritation, iris discolouration, foreign body sensation, keratitis, visual disturbance, headache, raised liver enzymes, hypertension, conjunctival oedema.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Precautions: Renal, respiratory or moderate to severe hepatic impairment, risks of macular oedema. Warn patient of possible eyelash growth, darkening of eyelids and change in eye colour which may not be noticeable until several months. Monitor for keratopathy in patients with dry eyes. Contact lenses (leave interval of 15 min before reinserting). Inflammatory, neovascular, angle closure or congenital glaucoma. Risk factors for bradycardia, hypotension. History of significant ocular viral infection, uveitis or iritis.
Advice to Patient
Wash hands properly before use. Remove contact lenses before use. Wait 15 minutes before reinserting. To instill medication into eyes, tilt head back, look upward, instill drops into the eye. Close eye and roll eyeball in all directions. Do not blink for 30 seconds. Apply gentle pressure to inner corner of eye for 30 seconds. Contact prescriber if there is eye pain; eye infection; changes in vision; swelling, itching in eyes; no improvement or worsening of condition. Avoid touching tip of applicator with eye. Do not share medication with anyone else. Wait 5 minutes before instilling any other medication.

Onset: 4 hrs. Duration: 1.5 hrs. Absorption: Well absorbed into the eye (cornea). Also absorbed systemically. Metabolism: Hydrolyzed to its active form in the eye. Metabolized via oxidation after systemic absorption. Half-life elimination: 45 mins. Time to peak: 10 mins. Excretion: Urine (67%). Faeces (25%).
