Atorvastatin and Fenofibrate

Hypolipidaemic/Anti-atheroma · Fibric acid/HMG-CoA reductase inhibitor

Indications

Mixed hyperlipidaemia.

Dosage

ORAL: Adults: 1 tab daily. Children: Not recommended.

Adverse Effects

Abnormal LFTs, gastritis, abdominal pain, back pain, leg pain, joint pain, burning feet, body ache, numbness in legs, rhinitis, constipation, flatulence, dyspepsia, headache, nausea, myalgia, asthenia, diarrhoea, insomnia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Hepatic or severe renal dysfunction, unexplained persistent liver function abnormality, unexplained persistent elevations of serum transaminases exceeding 3 times the upper limit of normal. Preexisting gallbladder disease. Precautions: Uncomplicated myalgia, history of liver disease, renal impairment. Discontinue treatment in case of acute, serious condition suggestive of myopathy, or having risk factor predisposing to the development of renal failure secondary to rhabdomyolysis, or presence of gallbladder stones.

Interactions

Digoxin, colestipol, erythromycin, cyclosporine, azole antifungals/niacin, antacids, propranolol, oral anticoagulants, oral contraceptives containing norethindrone and ethinyloestradiol.

Advice to Patient

Take with meals.

Parent drug and equipotent metabolites: Approx. 30%. Half-life elimination: Parent drug: Approx. 14 hrs. Equipotent metabolites: 20-30 hrs. Time to peak, serum: 1-2 hrs. Excretion: Bile. Urine. FENOFIBRATE: Absorption: Increased when taken with meals. Metabolism: Fenofibrate: Tissue and plasma. Fenofibric acid (active form): Hepatic or renal. Bioavailability: Fenofibric acid: Approx. 81%. Half-life elimination: Fenofibric acid: Mean: 20 hrs. Time to peak: 2-8 hrs. Excretion: Urine (approx. 60%). Faeces (25%).
