Atenolol and Chlorthalidone
Betablocking Agent · Beta-blocker/Diuretic
Indications
Management of hypertension, angina pectoris. Cardiac dysrhythmias. Treatment of myocardial infarction. Early and late intervention after myocardial infarction.
Dosage
Hypertension: Adults: 50 or 100 mg once daily. Angina: Adults: Usually 100 mg once daily or 50 mg twice daily. Children: Not recommended.
Adverse Effects
Cold extremities, muscular fatigue, bradycardia. Sleep disturbance rarely seen. Rashes and dry eyes have been reported with beta-blockers, consider discontinuance if they occur. Nausea, dizziness, rarely idiosyncratic reactions such as thrombocytopenia and leucopenia.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Positive evidence of risk: Use only when no safer alternative exists for a serious problem. Lactation: Caution advised or effect undetermined. Contraindications: Second or third degree heart block, cardiogenic shock. Precautions: Poor cardiac reserve. Avoid in overt heart failure. Anaesthesia. Chronic obstructive airways disease or asthma. Withdrawal of beta-blocking drugs should be gradual in patients with ischaemic heart disease. Withdrawal of clonidine. Renal impairment. Sensitivity to chlorthalidone. Diuretics may cause hyperuricaemia. Diabetes. If symptoms attributable to slow heart rate, reduce dose. Measure potassium levels.
Interactions
Verapamil, lithium, digitalis, or Class 1 antidysrhythmic agents.

Antihypertensive effect: Oral: 24 hrs. Absorption: Oral: Rapid, incomplete (approx. 50%). Metabolism: Limited hepatic. Half-life elimination: Beta: Newborns (<24 hrs age) born to mothers receiving atenolol: Mean: 16 hrs, up to 35 hrs. Children and Adolescents 5-16 yrs: Mean: 4.6 hrs. Range: 3.5-7 hrs. Adults: Normal renal function: 6-7 hrs, prolonged with renal impairment. ESRD: 15-35 hrs. Time to peak, plasma: Oral: 2-4 hrs. Excretion: Faeces (50%). Urine (40%).
