Indications

Rheumatoid arthritis, cryopyrin-associated periodic syndromes (CAPS).

Dosage

Rheumatoid arthritis: Adults: SC: 100 mg/day daily. Cryopyrin-Associated Periodic Syndromes: Neonatal-Onset Multisystem Inflammatory Disease (NOMID): SC: 1-2 mg/kg daily. Max: 8 mg/kg daily.

Administration

Administered via SC route. Stability: Store at 2°C-8°C. Do not freeze or shake. Protect from light.

Adverse Effects

RA: Worsening of RA, upper RTI, headache, nausea, diarrhoea, sinusitis, arthralgia, flu like-symptoms, abdominal pain. NOMID: Headache, vomiting, arthralgia, pyrexia, nasopharyngitis.

Risk Factor (Pregnancy/Lactation)

Pregnancy: No evidence of risk in humans. Lactation: Caution advised or effect undetermined. Contraindications: Neutropaenia (ANC <1.5 x 109/l). Precautions: Before starting treatment, neutrophil counts must be assessed and during treatment, monthly for 3 mths, and thereafter quarterly for a period up to 1 yr. Elderly. Renal impairment.

Interactions

TNF blocking agents, live vaccines (avoid).

Advice to Patient

Administer the dose at same time each day. Avoid contact with people who have infections that may spread to others (such as chicken pox, measles, flu etc.). Wash your hands well to prevent the spread of infection.

Bioavailability: 95% Half-life elimination: RA: 4-6 hrs. NOMID: 5.7 hrs. Time to peak, plasma: RA: 3-7 hrs. Excretion: Urine.
