Indications

Mixed-infections involving beta-lactamase-producing staphylococci.

Dosage

ORAL: Adults: 500 mg (Amoxicillin 250 mg & flucloxacillin 250 mg) every 6 hr, dose doubled in severe infections. Children: Above 10 yr, adult dose. Under 10 yr, half adult dose, the dose doubled in severe cases. INJECTION: Adults: 500 mg (Amoxicillin 250 mg & flucloxacillin 250 mg) every 6 hr, dose doubled in severe infections. Children: 2-10 yr, half adult dose, the dose doubled in severe cases.

Administration

Administer via slow IV/IM inj.

Adverse Effects

Hypersensitivity reaction, GI upset. Rarely hepatitis, cholestatic jaundice, haematological and CNS effects, pseudomembranous colitis, increased INR. Stevens-Johnson syndrome, toxic epidermal necrolysis.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Risk cannot be ruled out. Lactation: Caution advised or effect undetermined. Contraindications: Hypersensitivity to penicillins, cephalosporins or other beta-Iactams. History of flucloxacillin associated jaundice/hepatic dysfunction. Precautions: Renal impairment (CrCl<30 mL/min). Risk of crystalluria with high doses, all should be well hydrated. Monitor renal, hepatic and haematological function in prolonged treatment.

Interactions

Anticoagulants, allopurinol, probenecid.

Advice to Patient

Take with or without meals. To prevent stomach upset, take with meals. Contact prescriber if there is any allergic reaction (rash, hives, difficulty in breathing, swelling on face/lips/throat/tongue), vaginal itching/discharge, white patches on tongue, nausea, vomiting. Complete full course of therapy even if you feel better before completion of course.

AMOXICILLIN: Absorption: Oral: Immediate-release: Rapid with or without food. Extended-release: Rate of absorption is slower compared to immediate-release formulations; food reduces the rate but not extent of absorption. Half-life elimination: Adults: Immediate-release: 61.3 mins. Extended-release: 90 mins. Time to peak: Cap, oral susp: 1-2 hrs. Chewable tab: 1 hr. Extended-release: 3.1 hrs. Excretion: Urine. FLUCLOXACILLIN: Absorption: Absorbed rapidly from the GI tract. Absorption is reduced by the food in the stomach. Metabolism: Hepatic. Bioavailability: Oral: 50%-70%. Half-life elimination: 1 hr. Prolonged in neonates. Time to peak, plasma: Oral: 1 hr. IM inj: 30 mins. Excretion: Urine (mainly). Faeces (small amounts of dose).
