Indications

Substitution therapy for treatment of essential hypertension.

Dosage

ORAL: Adults: One tablet once a day, preferably in the morning.

Adverse Effects

Swelling of ankles or feet, dizziness, difficulty in breathing, increased heartbeat, redness of face/neck/arms/chest, muscle pain, abdominal pain, somnolence, heartburn, hypokalaemia, somnolence, headache, flushing, nausea, maculopapular rashes, oedema, fatigue, insomnia, mood changes, depression, syncope, hypoesthaesia, paresthesia, tremor, dysgeusia, visual disturbance, tinnitus, hypotension, dyspnoea, rhinitis, vomiting, dyspepsia, dry mouth, constipation, purpura, alopecia, skin discolouration, hyperhidrosis, pruritus, rash, exanthema, arthralgia, myalgia, muscle cramps, back pain, micturition disorder, nocturia, increased urinary frequency, impotence, gynaecomastia, chest pain, asthenia, pain, malaise, weight increase, weight decrease.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Hypotension, cardiogenic shock. Severe renal failure, hepatic encephalopathy or severe hepatic impairment. Hypokalaemia. Obstruction of the outflow tract of the left ventricle, Haemodynamically unstable heart failure after acute myocardial infarction. Precautions: Cardiac failure, renal and hepatic impairment.

Interactions

Itraconazole, rifampin, amiodarone, dexamethasone, carbamazepine, lithium, dantrolene, grapefruit, NSAID, COX-2 selective inhibitors, ACE inhibitors, high dose salicylic acid, digitalis, baclofen, allopurinol CYP3A4 inhibitors, CYP3A4 inducers, simvastatin. To be taken into consideration: Potassium sparing diuretics, metformin, iodinated contrast media, imipramine, neuroleptics, calcium salts, cyclosporine, tacrolimus, corticosteroids, other medicinal products with antihypertensive properties.

Advice to Patient

Take with or without meals. Swallow whole, do not chew/crush/break it. Take dose at fixed time daily. Avoid grapefruit juice. If a dose is missed, take it as soon as possible. Skip the missed dose if it is time for the next scheduled dose. Be cautious while driving and doing other tasks that require alertness. Contact prescriber if there is dizziness, headache, swelling of hands/feet.

AMLODIPINE: Onset of action: Antihypertensive effect: Significant reductions in blood pressure at 24-48 hrs after first dose. Duration: Antihypertensive effect: At least 24 hrs. Discontinuation after 6-7 wks of treatment: Duration extends to at least 72 hrs. Absorption: Well absorbed. Metabolism: Hepatic. Bioavailability: 64%-90%. Half-life elimination: Terminal (biphasic): 30-50 hrs. Time to peak, plasma: 6-12 hrs. Excretion: Urine. INDAPAMIDE: Absorption: Rapid and complete. Metabolism: Extensively hepatic. Bioavailability: 93%. Half-life elimination: Biphasic: 14 and 25 hrs. Time to peak: 2 hrs. Excretion: Urine (Approx.70%). Faeces (23%).
