Indications
Serious infections due to amikacin sensitive gram negative organisms. Known or suspected staphylococcal infections.
Dosage
Adults: 15 mg/kg IM or IV daily in two divided doses for up to 10 days; total max 15 gm. Children: Neonates, initially 10 mg/kg then 15 mg/kg daily in two divided doses; others same as adult. Bone & Joint Infections: IM/IV: Adults, Children & Older Infants: 7.5 mg/kg every 12 hr or 5 mg/kg IM/IV every 8 hr. Max: 1.5 gm/day. Newborns: Loading Dose: 10 mg/kg. Maintenance: 7.5 mg/kg every 12 hr. Lower Respiratory Tract Infections: IV: Adults, Children & Older Infants: 7.5 mg/kg every 12 hr or 5 mg/kg IM/IV every 8 hr. Max: 1.5 gm/day. Administration
IM: Administer IM injection in large mass. Do not mix with other drugs, administer separately. IV: The quantity of infusion solution should be sufficient to infuse amikacin over 1 to 2 hours in infants and 30-60 min children and adults; at a final concentration not to exceed 5 mg amikacin/ mL. Compatibility: Stable for 24 hours at room temperature and 2 days at refrigeration when mixed in D5W, D5¼ NS, D5½ NS, NS, LR. Adverse Effects
Ototoxicity, nephrotoxicity, neuromuscular blockade.
Risk Factor (Pregnancy/Lactation)
Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Myasthaenia gravis. Precautions: Ensure adequate hydration. Renal impairment. Hearing or vestibular damage. Perform audiogram before and during treatment. Parkinsonism. Elderly.
Interactions
Furosemide, ethacrynic acid, anaesthetics, neuromuscular blocking drugs, cephalosporins, bisphosphonates, platinum compounds, diuretics, other neurotoxic, ototoxic or nephrotoxic agents.
Advice to Patient
Take sufficient amount of fluids (2-3 litres per day) to maintain hydration, unless restricted by the prescriber. Contact prescriber if you experience changes in hearing, change in keeping balance, vertigo, feeling of fullness in head, pain/tingling/numbness of any body part, change in urinary pattern, white plaques in mouth, vaginal discharge, unhealed sores, sore throat, unusual chills, fever, pain/redness/swelling at injection site.

Absorption: IM: Rapid. Oral: Poorly absorbed. Half-life elimination (renal function and age dependent): Infants: Low birth weight (1-3 days): 7-9 hrs. Full-term >7 days: 4-5 hrs. Children: 1.6-2.5 hrs. Adolescents: 1.5 ± 1 hr. Adults: Normal renal function: Approx. 2 hrs. Anuria/end-stage renal disease: 17-150 hrs. Time to peak, serum: IM: 60 mins. IV: Within 30 mins following a 30-min infusion. Excretion: Urine.
