Indications
Treatment of functional symptoms of benign prostatic hyperplasia.
Dosage
Adults: 10 mg once daily or 2.5 mg 3 times daily. Elderly: 5 mg morning and evening. Children: Not recommended. First dose may cause collapse due to hypotensive effect (therefore should be taken on retiring to bed). Patients should be warned to lie down if symptoms such as dizziness, fatigue or sweating develop and to remain lying down until they abate completely.
Adverse Effects
Vertigo, dizziness, headache, Gl upset. Orthostatic hypotension, syncope, tachycardia, palpitations, chest pain, fatigue, drowsiness rash, pruritus, flushes, oedema, asthenia, malaise, dry mouth.
Risk Factor (Pregnancy/Lactation)
Pregnancy: No evidence of risk in humans. Lactation: Contraindicated or not recommended. Contraindications: History of orthostatic hypotension. Severe hepatic impairment. Sust-release preps: hepatic or severe renal impairment (CrCl <30mL/min). Precautions: QT prolongation, hypertension; monitor BP regularly, particularly at start of treatment. Coronary insufficiency; discontinue if angina deteriorates. Withdraw 24 hours before anaesthesia. Renal or hepatic impairment. Cerebral circulatory disorders. Risk of priapism; warn patients to seek immediate medical assistance. Cataract surgery. Elderly.
Interactions
Other alpha-blockers, antihypertensives, calcium antagonists, nitrates, potent CYP3A4 inhibitors, drugs that prolong QT interval.
Advice to Patient
Take immediately after the evening meals. Do not chew or crush tablets, swallow whole. Take as directed, at the same time each day. Contact prescriber if there is unusual chest pain, difficulty in breathing. Change position slowly when climbing stairs and from lying or sitting to standing position.

Absorption: Reduced 50% under fasting conditions. Metabolism: Hepatic. Bioavailability: 49% following a meal. Half-life elimination: 10 hrs. Time to peak, plasma: 8 hrs following a meal. Excretion: Faeces (69%). Urine.
