Alectinib

Indications

Anaplastic lymphoma kinase (ALK)-positive metastatic non-small cell lung cancer (NSCLC).

Dosage

Adults: 600 mg orally twice daily. Administer until disease progression or unacceptable toxicity. In case of intolerance, reduce dose to 450 mg orally twice daily. If further reduction is required, reduce to 300 mg orally twice daily. If still not tolerated, discontinue. Children: Not recommended.

Adverse Effects

Fatigue, constipation, oedema, myalgia and anaemia.

Risk Factor (Pregnancy/Lactation)

Pregnancy: Contraindicated in pregnancy. Lactation: Contraindicated or not recommended. Contraindications: Interstitial Lung disease (ILD)/Pneumonitis. Severe myalgia, elevated CPK. Precautions: Severe hepatic impairment. Severe renal impairment.

Advice to Patient

Take with meals. Do not open or dissolve the contents of the capsule. Swallow whole capsule. If a dose is missed or vomiting occurs after taking a dose, take the next dose at the scheduled time. Inform prescriber if you experience any of the following symptoms: Change in urine colour, reduced urine output, swellings in the legs and feet, dizziness, lightheadedness, syncope, persistent muscle pain/tenderness or weakness. Avoid prolonged sun exposure during treatment and for at least 7 days after discontinuation. Use a broad spectrum ultraviolet A(UVA)/ultraviolet B (UVB) sunscreen and lip balm (SPF=50) to help protect against potential sunburn.

Absorption: High-fat, high-calorie meal increases absorption. Bioavailability: Absolute: 37% (under fed conditions). Metabolism: Hepatic. M4 is major active metabolite. Half-life elimination: Alectinib: 33 hrs. M4: 31 hrs. Time to peak, serum: 4 hrs (under fed conditions). Excretion: Faeces (98%). Urine.
