Indications

Rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, ankylosing spondylitis, Crohn’s disease, ulcerative colitis, plaque psoriasis, hidradenitis suppurativa (HS).

Dosage

SC: Rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis: Adults: 40 mg every other week. Patients with RA not taking MTX may increase dosage to 40 mg every week or 80 mg every other week. Children: Not recommended. Juvenile Idiopathic Arthritis: Children: =2 yrs: Body wt. 15 kg-<30 kg: 20 mg every other wk. Adults and children =30 kg: 40 mg every other wk. Crohn’s disease: Adults: 160 mg on day 1 (given in 1 day or split over 2 consecutive days), 80 mg on day 15 and 40 mg every other week starting on day 29. Children =6 yrs: 17 kg- <40 kg: Day 1: 80 mg. Day 15: 40 mg. Day 29: 20 mg every other week. =40 kg: Day 1: 160 mg (single dose or split over 2 consecutive days). Day 15: 80 mg. Day 29: 40 mg every other wk. Ulcerative Colitis: Adults: 160 mg on day 1 (given in 1 day or split over 2 consecutive days), 80 mg on day 15 and 40 mg every other wk starting on day 29.…

Administration

Administered subcutaneously via a prefilled syringe or auto-injector. Allow the prefilled syringe or auto-injector to sit at room temperature for a specified period (typically 15-30 mins) before use. Do not use external heat sources to warm the medication. Inject into the front of the thighs or the stomach (abdomen), at least 2 inches away from the belly button. Do not use areas with skin that is tender, bruised, red, or hard. Rotate injection sites to avoid skin irritation, keeping each new spot at least 1 inch away from previous sites. Adverse Effects

Infections (e.g. upper respiratory, sinusitis), inj site reactions, headache, rash.

Risk Factor (Pregnancy/Lactation)

Pregnancy: No evidence of risk in humans. Lactation: May be used in breast feeding. Contraindications: Active infection. Precautions: HBV carriers, demyelinating disease, CHF, lupus. Elderly.

Interactions

Abatacept, anakinra, live vaccines.

Advice to Patient

Rotate injection sites (abdomen or thigh) at least 1 inch from previous spots to help prevent injection site reactions such as soreness, redness, or skin damage. Contact a healthcare provider promptly if signs of infection, such as fever or persistent cough, develop. Immediately report symptoms such as numbness/tingling, vision problems, severe weakness, or signs of heart failure.
